Fees & Insurance

At this time I am not filing insurance. Please be prepared to pay for your session fee in full during your appointment. Accepted forms of payment include cash, check, and credit card.

What should I bring to the initial appointment?

Please bring a completed nutrition assessment form.
List of medications, vitamins, and supplements you are currently taking.

What if I need to change my appointment?

To change or cancel an appointment, I require at least a 48-business hour notice. This will help me to schedule those waiting for appointments and for you to avoid being charged for the time that was reserved for you. Clients who cancel without a 48-hour notice or do not attend their appointment will incur a minimum of a $20.00 cancellation/no show fee.

Where is your office?

If you are a patient of Dr. Suggs, I can see you either at his office building at 705 Bank Street Decatur, AL or at the Madison Surgery Center located at 460 Lanier Road Madison AL 35758 suite 202A.

What are your office hours?

Wednesdays from 8 am to 5 pm I see Dr. Suggs’ patients by appointment at 705 Bank Street Decatur, AL. Thursdays from 12 pm to 5 pm I see Dr. Suggs’ patients by appointment at 460 Lanier Road Madison AL 35758.
